REGULAR SCHOOL

Students may be admitted upon presentation of the following credentials:

Pre-school  

1. Birth Certificate  (original and photocopy)

2. Baptismal Certificate (original and photocopy)

3. Duly accomplished Application Form

Elementary

1. Form 138 (report card) duly signed by the authorities from the last school attended (original and photocopy)

2. Birth Certificate (original and photocopy)

3. Baptismal Certificates (original and photocopy)

4. Certificate of Good Moral Character

5. Duly accomplished Application form

6. Results of the entrance examination (consisting of mental ability and academic achievement tests)

High School

1. Form 138 (report card) duly signed by the authorities from the last school attended (original and photocopy)

2. Birth Certificate (original and photocopy)

3. Baptismal Certificates (original and photocopy)

4. Certificate of Good Moral Character

5. Duly accomplished Application form

6. Results of the entrance examination (consisting of mental ability and academic achievement tests)

7. Certificate of Good Moral Character from previous school

8. Two (2) recommendation letters either from the Principal, Guidance Counselor or Class Adviser of previous school

Additional Requirements for Foreign Students:

Refer to above requirements for pre-school, elementary and high school, then add the following for submission:

1. Study permit from the Department of Education

2. Department of Education Evaluation

3. Passport number, date and place of issuance

NOTE:

• Student-applicants who are able to complete the above requirements will be scheduled for entrance testing. Results of the entrance test can be obtained immediately the next day.
• All requirements to be submitted and put inside a long brown envelope.
• Passers of the entrance exam are interviewed and if accepted, are oriented together with their parents or guardian/s by the panel of the Committee on Admissions.
• Final check on the original report card is done by Committee on Admissions.
• The School reserves the right not to admit any student with no records to show, or with sub-par records, either in academic or behavioral areas, from the previous school.
