4. Certificate of Good Moral Character<\/p>\n

5. Duly accomplished Application form<\/p>\n

6. Results of the entrance examination (consisting of mental ability and academic achievement tests)<\/p>\n

High School<\/span><\/p>\n

1. Form 138 (report card) duly signed by the authorities from the last school attended (original and photocopy)<\/p>\n

2. Birth Certificate (original and photocopy)<\/p>\n

3. Baptismal Certificates (original and photocopy)<\/p>\n

4. Certificate of Good Moral Character<\/p>\n

5. Duly accomplished Application form<\/p>\n

6. Results of the entrance examination (consisting of mental ability and academic achievement tests)<\/p>\n

7. Certificate of Good Moral Character from previous school<\/p>\n

8. Two (2) recommendation letters either from the Principal, Guidance Counselor or Class Adviser of previous school<\/p>\n

Additional Requirements for Foreign Students:<\/span><\/p>\n

Refer to above requirements for pre-school, elementary and high school, then add the following for submission:<\/p>\n

1. Study permit from the Department of Education<\/p>\n

2. Department of Education Evaluation<\/p>\n

3. Passport number, date and place of issuance<\/p>\n

NOTE:<\/span><\/p>\n

  • Student-applicants who are able to complete the above requirements will be scheduled for entrance testing. Results of the entrance test can be obtained immediately the next day.<\/li>\n
  • All requirements to be submitted and put inside a long brown envelope.<\/li>\n
  • Passers of the entrance exam are interviewed and if accepted, are oriented together with their parents or guardian\/s by the\u00a0panel\u00a0of the Committee on Admissions.<\/li>\n
  • Final check on the original report card is done by Committee on Admissions.<\/li>\n
